"Take Me Home" Registry
The Montclair Police Department offers a "Take Me Home" questionnaire designed to assist those who require special assistance in emergency situations. The information provided by you on this form will be held in the strictest confidence and will be shared only as necessary to ensure the safety of individuals in case of an emergency. Questionnaire information will be entered into the Montclair Police Department’s dispatch computer database so that, in the event of an emergency, the Police, Fire and Emergency Medical Services of the Township of Montclair can better serve you and your loved ones.

Operation Blue Angel
The Montclair Police Department, Montclair Fire Department, and Montclair Ambulance Unit have collaborated through Lifelong Montclair, the Township’s aging in place initiative, to give our senior residents more peace of mind. “Operation Blue Angel” is intended to provide Police, Fire and Emergency Medical Services (EMS) access to a residence in the event of an emergency where the resident is unable to open the door.
Thanks to funding from the Partners for Health Foundation, eligible residents will be provided with a free lock box. The key, provided by the applicant, will be stored in the secure lock box near the entrance to their residence. The access code to the lock box will only be available to first responder personnel and used only during an emergency.
The Community Service Unit (CSU) supervisor will assign an officer from the unit to respond to the applicant residence to recode the lockbox. Whenever the lock box is entered during the hours when the CSU is not available, dispatch will send an email and voicemail to the supervisor of the CSU advising him/her of the incident. The applicant and/or the emergency contact person listed on their application will be provided with the new code. The new code will transmit to dispatch via email and logged by the Lock Box Coordinator.
To be eligible to enroll in “Operation Blue Angel” lock box program, Montclair residents should meet the following requirements: they must be 55 years or older or have medical conditions that could lead to incapacitation and live alone or are alone for extended periods of time on a regular basis.

The New Jersey State Police (NJSP) Firearms Investigation Unit has mandated the statewide activation of the Firearms Application and Registration System (FARS).
The activation of FARS will allow firearms applications to be created and submitted online.

Temporary Handicapped Parking Placard Application
Temporary placards are issued for six months, with one six-month renewal allowed.
To apply for a temporary parking placard you must:
- Download and complete the Temporary Placard Application [pdf] or visit the Montclair Police Department at 647 Bloomfield Avenue to obtain the application .
- Have a qualified medical practitioner certify that you are qualified for a temporary placard; see Qualifications for medical certification information.
- Submit the completed application to the Township Police Department with a check or money order made payable to the NJ Motor Vehicle Commission in the amount of $4.00.
- After review and upon approval by the Chief of Police, the department will issue your temporary placard. TEMPORARY PLACARD APPLICATIONS MUST BE SUBMITTED IN PERSON TO THE TOWNSHIP CHIEF OF POLICE.
DO NOT SUBMIT the payment or the completed application to the MVC.
